Follow through and Phase Out
Teacher and parent behavior efforts should be discussed with any
outside individuals who may also be working with the child, such
as therapists.
Follow up meetings need to be convened periodically to review the
implementation of the plan and the effectiveness. This may need
to occur every 3 to 6 weeks. Changes to the plan or to the interventions
need to be discussed at that time (e.g. phasing out rewards).
Once the child has effectively reached the goals in the Behavior
Management Plan, student feedback and monitoring should be phased
out on a gradual basis. If the support tools, such as Self-Monitoring
Sheets or desk charts, are removed
too quickly, the child may revert to the problem behaviors. Even
with gradual removal of rewards and reminders, some children may
need continual reminders, such as schedules and visuals, to help organize activities and control behaviors.

Behavior Management Plan, follow through for CW includes:
1. The family has indicated they are working with a therapist on
self-regulation and behavior management at home. Therapy is focusing
on helping CW recognize feelings and select appropriate responses.
The therapist will consult and work with CW's instructors to help
maintain a consistent management plan and interventions.
2. Bi-weekly emails with CW's therapist will occur to review CW's
progress and make necessary adjustments to his behavior plan.
3. A meeting with parents, teachers, and therapists will be convened
in four weeks to review progress and adjust the behavior plan if
necessary.
4. When CW reaches his goals, a method for phase out will be determined
based on the time he took to learn the behaviors and his dependency
on each particular feedback technique. |
